For the photographer (that I adore and it is of my favourite here on DA) impressive work, I like the use of the flash (colored I suppose with a CTO) and also the perspective and angle of view.
In general, the scene seems very well managed, although little parts of arm much exposed by the flash are very burned and I do not like them very much, perhaps it would be useful to set the flash output a stop lower, but that is just my opinion. Another thing I do not like much is the blur on the front hand, also this is just a slight whim, because as is the photo is structured, the eye go automatically to the tattoo on the arm, then following the arm to the hand you get the focus, and this is unfortunately a bit annoying. Nevertheless, it is a beautiful photo and very intense.
